This was a pre fab home in NE PA.This was Inspected,and passed.Can you see what is wrong?

Is the Romex w/ jacket in the panel or just hanging in front? Top two breakers look a little loose too. Other than that, it's a prefab?

Looks like it was fed with a 4 wire feed, but the bond screw is installed.
